Job Description

Salary: $68k

The work schedule Monday - Friday 8:00 AM - 4:00 PM.

JOB DUTIES & RESPONSIBILITIES:
  • Directly supervises PCP Specialists including performance management training coaching and ongoing support to ensure consistency and quality in person-centered practices.
  • Reviews and approves Service and Provider Implementation Plans (SIP/PIP) to ensure alignment with individual goals regulations and best practices.
  • Accurately completes and submits required funding documents for services and plan approvals in the LTSS system ensuring compliance with timelines and program requirements.
  • Oversees and tracks the person-centered planning process for people supported ensuring accurate implementation documentation and reporting within the iCareManager (iCM) system.
  • Completes required pre-planning tools such as the Detailed Service Authorization Tool (DSAT) and support ongoing training as per regulatory requirements.
  • Monitors and maintains compliance of individual files in iCareManager (iCM) with state federal and agency standards maintaining accountability for accuracy timeliness and regulatory adherence.
  • Collects coordinates and reviews all relevant assessments and documentation required for PCP development including HRST SIS NPOC and behavior plans maintaining responsibility for accuracy completeness and alignment with individual needs.
  • Completes required pre-planning tools such as the Detailed Service Authorization Tool (DSAT) and supports ongoing training as per regulatory requirements maintaining accountability for accuracy compliance and ensuring staff are equipped to achieve positive outcomes.
  • Ensures the DSAT and all other required documentation are accurately completed and submitted to the DDA within required timeframes maintaining accountability for compliance with regulatory standards timeliness and the integrity of submitted information.
  • Completes 90-day reviews and ensures effective ongoing monitoring of plan implementation maintaining accountability for compliance with standards timely follow-up and achieving measurable outcomes aligned with individual goals.
  • Collaborates with the Program Director or designee to review and monitor service authorizations in LTSS for accuracy and compliance maintaining responsibility for timely resolution of discrepancies and ensuring individuals receive appropriate services and supports.
  • Tracks the flow of plans through the LTSS system and responds to requests for additional information as necessary ensuring accountability for timeliness accuracy and continuity of services to achieve intended outcomes.
  • Conducts Council for Quality and Leadership (CQL) Personal Outcome Measure (POM) interviews and assessments as needed ensuring accurate completion compliance with CQL standards and accountability for maintaining the integrity reliability and usefulness of assessment results.
  • Utilizes the iCareManager system to track report and analyze PCP progress and outcomes maintaining accountability for accurate data management and ensuring information is used to support compliance informed decision-making and achievement of individual goals.
  • Supports Direct Support Professionals (DSPs) through training and coaching to ensure proper implementation of individual outcomes and supports maintaining accountability for staff competency and promoting successful achievement of person-centered goals.
  • Collaborates with the finance department to identify and resolve LTSS billing or authorization discrepancies maintaining accountability for accuracy compliance with funding requirements and timely resolution to ensure continuity of services and proper reimbursement outcomes
  • Initiates and follows up on funding requests with the Coordination of Community Services (CCS) for new or modified services ensuring accountability for timeliness accuracy and successful acquisition of resources to meet individual needs and outcomes

EDUCATION & EXPERIENCE REQUIREMENTS:

  • Bachelors degree in the field of psychology human services or education or a related field.
  • Three (3) years of professional experience delivering community-based services to people with intellectual and developmental disabilities some experience in billing and finance a Experience may be substituted for education on a 2:1 basis.
  • Knowledge of Code of Maryland Regulations (COMAR) & Home and Community Based Services (HCBS) Final Rule waiver and service definitions and regulations guidelines procedures &

  • Requires proficiency in using computer systems and software to perform job function including but not limited to basic Windows PC web browsing (i.e. Chrome Internet Explorer etc.) and Microsoft Outlook. Knowledge of other Microsoft Office applications such as Word Excel PowerPoint Teams and OneDrive is preferred.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

  • Flexibility may be required to work beyond standard hours including weekends and holidays as needed.
  • Hybrid work options are available contingent upon business needs and performance expectations in alignment with the Agencys policy upon successful completion of a 6-month introductory period.
  • Adjusting or moving objects up to 25 all directions which may include lifting carrying pushing pulling or repositioning items as necessary to perform job duties.
  • Moving about to accomplish tasks or transitioning between different work sites which may involve walking standing or traveling to various locations as required by the role.
  • Engaging in communication with others to exchange information which may involve speaking listening writing or using electronic communication methods to ensure clear and effective understanding. This includes both verbal and non-verbal interactions with colleagues clients or external parties.
  • Hearing and vision abilities with or without correction sufficient to drive safely and effectively observe and communicate with others.
  • Driving (Agency and/or personal vehicle) is required for this position. Driver must be at least 21 years of age with a minimum of 2 years of driving experience and a valid drivers license issued by the state of residence. There can be no state issued restrictions on the licenses that would impede drivers ability to operate the vehicle as required by the Agency and there must not be more than three (3) points on their driving record.
