About the role

Join Villa Maria Catholic Homes and lead a dedicated team delivering exceptional home care services to our community members. This full-time position offers the opportunity to make a meaningful difference in peoples lives while advancing your leadership career in healthcare.

As our Clinical Team Leader youll guide and support our home care team in providing compassionate person-centered care. Youll work closely with clients families and healthcare professionals to ensure the highest standards of care delivery while fostering a positive collaborative team environment.

• Provide clinical leadership and mentorship to home care staff
• Conduct comprehensive client assessments and care planning
• Ensure compliance with regulatory standards and quality frameworks
• Collaborate with families healthcare professionals and multidisciplinary teams
• Support staff development through training and professional guidance
• Monitor and evaluate care outcomes to drive continuous improvement

About you

• Experience in home community and restorative care settings (essential)
• Current AHPRA registration as a Registered Nurse (RN)
• Willingness to travel regionally once per fortnight
• Current Victorian drivers licence and National Police Check clearance.
• Proven experience in case management assessment care planning and participant budget management.
• Strong written verbal and numerical communication skills & proficiency in MS Office.
• Knowledge of Aged Care Reforms Quality Standards and community aged care services.
