What is Nursing in Adult Foster Care at The Arc of Opportunity

Our Adult Foster Care (AFC) Program is a medicaid-funded initiative that enables individuals with disabilities to live with caring providers in a safe and nurturing environment. The program provides comprehensive case management and nursing oversight. The nursing staff in our AFC program monitors the health status of individuals supported in our Adult Foster Care program and acts as a resource for all program staff and home.


What Does the AFC Nurse Case Manager Do

  1. Coordinate medical information for admission and provide a comprehensive nursing assessment for the intake process when required by individual medical condition(s).
  2. Provide participant specific medical training for home care providers and nursing case training as needed.
  3. Monitor medical care to ensure that consumers receive quality care treatment and respect in the community.
  4. Maintain Individual Confidential Files and monitor placements to ensure compliance with all applicable regulations.

Qualifications:

  1. Current licensure to practice as a Registered Nurse by the Commonwealth of Massachusetts Board of Registration of Nursing
  2. Associates or Bachelors in relevant field is preferred.
  3. A minimum of two years prior experience with elderly or individuals with disabilities
  4. Proven ability to utilize electronic health record systems to monitor compliance (currently iCentrix and HCSIS)
