SCOPE OF WORK:
The incumbent of this position is responsible for providing intensive case management services and assistance with unhoused families seeking to enroll in the Family Housing Solutions program and family shelter at Visalia Navigation Center. Staff will also assist in the planning and implementation of Grant Program and ensure that the tasks and objectives of specific program grant performance service goals are being met. Staff are responsible for screening assessing completing risk evaluations planning support services and supporting self-sufficiency. Families will be provided case management services for up to 2 years.
ACCOUNTABILITIES:
- Works closely with the Senior Program Coordinator and Director to establish guideline objectives staffing needs trainings start-up activities and support internal controls.
- Serves as an advocate for families experiencing homelessness presiding at the Visalia Navigation Center or in Tulare County.
- Evaluates needs of families enrolled in the program and track progress.
- Provides intense case management services for unhoused families to assist with gaining safe housing.
- Follows core principles of case management of building trust using evidence base practices and engaging families.
- Assists families with developing a self-sufficiency plan and provides referrals to support services that will assist with family stabilization.
- Plans and supports Navigation Center events to promote program goals and maximize benefits to residents at the center.
- Participates in outreach and promotion of the Family Housing Solutions program in the community with other community partners serving unhoused families.
- Follows and supports VNC operational safety procedures.
- Supports and fosters collaboration of activities among departments within CSET and other partners that may be used as resources for families enrolled in the Family Housing Solutions program.
- Supports and implements excellent customer service practices.
- Reports program support needs to appropriate leadership staff.
- Completes necessary reports regarding program services and progress.
- Performs other duties including but not limited to fulfillment of work schedules adherence to attendance policies and other applicable operating rules policies and procedures.
- Assist with quarterly reports as needed.
SKILLS & ABILITIES:
- Bilingual preferred.
- Ability to work late evenings or weekends as needed to meet program needs.
- Ability to travel throughout Tulare County to conduct needs assessments and meet with clients.
- Upholds company principles and practices to the highest degree and represent the agency in a professional manner.
- Ability to provide exceptional customer service that includes efficient precise and courteous communication.
- Knowledgeable of modern office procedures and methods and is proficient in use of computer Windows operating system including Microsoft Office applications (Word Excel and Access) and program data base.
- Must have excellent oral and written communication skills as well as ability to communicate effectively with low income persons other providers of human services community leaders and government officials.
- Ability to accurately record and deliver information meet deadlines and maintain confidentiality of restricted information.
- Ability to assist with narrative reports.
- Ability to perform successfully under moderately stressful conditions to respond immediately to crisis situations and to balance priorities within and between offices/departments/work sites.
- Ability to maintain personal composure tactfully handle difficult situations and interpret questions correctly; ability to behave in a friendly understanding helpful and professional manner with participants and program staff.
- Ability to advise and interpret on the application of policies procedures and standards to specific situations.
- Ability to explain demonstrate and clarify to others the understanding of the well-established principles practices policies and procedures.
- Implements and oversees the data collection activities. Uses data to conduct ongoing needs assessment.
- Coordinates data collection activities and reviews data from submission into several databases.
