Job Description

Job Title: Physical Therapist (Outpatient)

Specialty: Outpatient Physical Therapy serving a rural patient population.

Site: Woodrow Wilson Keeble Memorial Health Care Center (IHS)

Location: 100 Lake Traverse Drive Sisseton SD 57262.

Duration: 6 months contract with nine (9) additional 6-month extension.

Shift Schedule:

  • 8-hour shifts Monday Friday 8:00 a.m. to 4:30 p.m. 30-minute lunch; average 40 hours/week.
  • No weekend or holiday requirements

License /Certifications:

  • Current active unrestricted Physical Therapy license in at least one U.S. state or territory (submit copies of all current licenses active & inactive).
  • BLS
  • ACLS
  • NPI
  • Board Certification

Education:

  • Degree from an accredited Physical Therapy program.

Minimum Qualification/ Experience:

  • 1 year of experience as a Physical Therapist within the past 36 months.
  • Maintain continuing education (submit current CME/CE for past two years).

Responsibilities:

  • Provide comprehensive PT evaluations and treatments across a wide range of conditions (e.g. post-stroke musculoskeletal post-surgical acute/chronic pain; prenatal/postpartum as requested); establish goals with patients/families; reassess and document progress.
  • Consult with referring providers; develop and teach written home programs; participate in the Patient-Centered Medical Home (PCMH) model.
  • Document in the EHR; maintain timely accurate medical records; complete check-out with HIM on last workday; meet documentation timeliness metrics.
  • Uphold professional conduct cultural sensitivity patient rights and compliance with Federal/State/IHS/CMS and departmental policies; complete required trainings on time (privacy/HIPAA etc.).
  • Participate in committees as assigned (e.g. Medical Staff PRC QA Pain Management P&T OPD).
  • Adhere to clinic operations: follow defined visit lengths; use time clock; submit accurate timesheets and invoices; coordinate with PT Supervisor/COR on operational issues.
  • Safeguard government property and PHI; return badges at end of contract; report security concerns; comply with rules on weapons/firearms on facility grounds.
