The Wound/Skin Care Coordinator is responsiblefor coordinating implementing and monitoring a facility approved skin careprogram which includes prevention and treatment of skin problems related toimpaired skin integrity.
1.Represent Independence Care Center in a positive andprofessional manner.
2.Know and practice Residents rights.
3.Demonstrate a caring attitude in all interactions withresidents.
4.Abide by Independence Care Center Personnel Policiesand Procedures as outlined in the Employee Handbook.
5.Know and practice emergency policies and procedures.
6.Provide wound and skin care to all residents as orderedby their physician.
7.Must treat residents with respect and allow them tomaintain their dignity.
8.Do a weekly assessment of all residents with ulcers. Call family or DPOA weekly with update.
9.Complete and submit a weekly report to DON for review.
10.Complete Pressure Ulcer Risk Assessment on all newadmissions. Reassess quarterly thereafter.
11.Record Risk Assessment scores on flow sheet. Update asneeded.
12.Assess residents identified at risk at least bi-weeklywith documentation on flow sheet.
13.Provide pressure reduction devices for those residentsidentified at risk for development of pressure ulcers.
14.Update Wound and Skin Care Protocols as needed. Submitto DON for review and approval.
15.Monitor program for compliance.
16.Participate in Care Plan meetings as needed.
17.Work as LPN charge nurse if scheduled to do so.
18.Participate in QI Committee Meeting.
19.Responsible for assuring Resident safety.
20.All other duties as assigned.
QUALIFICATIONS:
1.Must be a graduate of a RN or LPN training program andhave current RN or LPN licensure in the State of Missouri.
2.Must be at least 18 years of age.
3.Must be able to read write and speak fluently inEnglish.
4.Must be able to communicate with residents and familiesin a pleasant and supportive manner. Also must be able to work with andcommunicate effectively with staff.
PHYSICAL AND SENSORY REQUIREMENTS:
1.Must be able to stand bend reach kneel carry lift75-100 lbs. and move intermittently during working hours. Must be willing andable to work a flexible work schedule when necessary.
2.To assist residents in a time of emergency must be ableto move quickly from one part of the building to another.
3.Must have good physical health with no communicablediseases present.
4.Must be able to demonstrate emotional stability inorder to cope with the mental and emotional stress of the position.
5.Must possess manual dexterity for operating a computerand other like equipment.
WORKING CONDITIONS:
Exposed to body fluids infection odors and behavior ofresidents.
