Provide flexible scheduling for patients offering house calls telehealth and in-clinic visits

Your Role with the Company:

Full-time position Flexible schedule and work/life balance
60-90 min visits specializing in peri menopause GI regenerative and functional medicine
Provider will see 3-5 patients/day home visits some clinic visits and telehealth
Nurse Practitioner will build up to carry a patient panel of 225 patients

Background Profile:

FNP / Family Nurse Practitioner APRN Adult Gerontology Nurse Practitioner
License and Independent practice authority in Illinois
2 years of Hormone experience required functional medicine experience desired
