Job Summary:
We are seeking a compassionate and dedicated Social Worker to join our Childrens Outpatient Program in Southgate MI. This full-time direct-hire position is ideal for professionals passionate about providing high-quality mental health care to children and families. The role involves delivering therapy services completing assessments and collaborating with a multidisciplinary team to support treatment and advocacy efforts.
Key Responsibilities:
Conduct intake assessments including Integrated Biopsychosocial evaluations and all required documentation (financial demographic etc.).
Collect and integrate data from multiple sources including the child family guardians and referring professionals.
Develop Person-Centered Treatment Plans collaboratively with clients and team members.
Provide individual family and group therapy sessions in-office with at least monthly face-to-face sessions per client.
Implement crisis planning and parent education coaching and training.
Coordinate and refer families to community resources and external services.
Collaborate with psychiatrists primary care physicians school personnel and other care providers.
Advocate for children and families to promote self-sufficiency and well-being.
Participate in clinical supervision team meetings and case consultations.
Maintain timely and accurate clinical records in accordance with agency policies.
Ensure licensure and certifications remain active and compliant.
Provide at least 23 hours of billable service weekly and be available to work four hours per week after 5:00 PM.
Complete 24 hours of annual training specific to child and adolescent care.
Perform additional duties as assigned.
Qualifications:
Education: Masters Degree in Social Work Psychology or Counseling Required
Licensure (Michigan): Must hold or be eligible for one of the following:
LLMSW LMSW
LLPC LPC
TLLP LLP
Fully licensed clinicians (LMSW LP or LPC) are preferred. Licensure must remain active and free of disciplinary action.
Must meet Michigan Department of Health & Human Services (MDHHS) requirements for Child Mental Health Professional (CMHP) within 120 days of hire.
Must pass MDHHS Central Registry and criminal background checks.
Experience: Minimum 1 year providing mental health services to minors and their families Required
Experience with Electronic Medical Records Preferred
Proficiency in Microsoft Office Suite and databases Required
