Position Highlights:
- Schedule: 7:30AM - 5:00PM4 days per week patient-facing with one half-day dedicated to administrative time (remainder of the day off).
- Provide comprehensive/primary care to chronically ill patients in an office setting
- Average of 14 patients per day
- Focus on reducing hospital readmission rates through proactive patient management and education
- Conduct thorough assessments diagnose conditions and prescribe appropriate treatments
- Robust APP onboarding program consistent with didactic curriculum specialty shadowing competency-based evaluations and mentorship

Minimum Requirements:
- Active NP or PA
- At least 5 years experience as APP
