Integrated Clinical Services provides quality health services for people who experience barriers to accessing care. The vision of Integrated Clinical Services is to provide services that are integrated and connected to support optimal health reducing wait times and optimizing access. Multnomah County Community Health Clinics provide comprehensive primary care and behavioral health services to diverse low-income uninsured and underinsured residents of Multnomah County. Each position in our Community Health Clinics plays an active role in advancing health equity and eliminating health disparities in our communities. The Peer Engagement Case Manager 2 (CM2) position performs a hybrid of complex case management and peer support functions for Multnomah County ICS patients as a member of the Transitions of Care - Behavioral Health team. The CM2 supports primary care patients who are transitioning from higher levels of care as well as patients in ongoing primary care with ICS whose diagnosed mental health and/or substance use disorders are exacerbated by social determinants of health stressors. Duties include case management in coordinating with multiple community partners to support smooth transitions in services for the patient so they can successfully connect to community resources. The CM2 is part of the primary care team and will work closely with clinic primary care team members as well as other team members of the Transitions of Care - Behavioral Health team to coordinate patient care planning.
As the Peer Engagement Case Manager 2 you will:
Provide complex clinical case management support for patients transitioning from acute care settings and referred from primary care teams or other role groups based on related mental health and/or substance use disorder issues.
Conduct a comprehensive needs assessment with the patient and/or other natural supports.
Review and monitor EPIC electronic health record (EHR) charts internal and external communications and case notes.
Maintain an ongoing caseload and track patient progress throughout their acute transition period and for those who need additional support post-acute transition
Maintain up-to-date notes and files per client event and interaction.
Establish and maintain effective relationships and up-to-date knowledge of psychiatric hospital services community mental health and behavioral health services substance use treatment services and community-based organizations that address housing food insecurity job training local governmental support programs and other resources.
Monitor quality of care provided and address problems in a timely manner working with other professionals and agencies as appropriate.
Research and take steps to find new community-based opportunities including visiting or meeting with partner organizations as needed.
Conduct task-oriented meetings in the community with referred patients as indicated to facilitate connection with needed resources and partner organizations.
Provide guidance and mentorship to Peer Support Apprentice interns.
Document all work with and on behalf of patients in the electronic health record.
Participate in quality improvement activities; utilize relevant Case Management data in decision making and planning.

Minimum Qualifications/Transferable Skills*:
Education/ training equivalent to an associates degree.
Three (3) to six (6) years of relevant experience that demonstrates the ability to perform the duties of the position.
Must have a Peer Support Specialist Certification issued by the Oregon Health Authority.
Must have a Qualified Mental Health Associate (QMHA) or (QMHA-R) certification through the Mental Health and Addiction Certification Board of Oregon (MHACBO).
Must pass a reference check.
Preferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/transferable skills to qualify. Qualified Mental Health Professional (QMHP) certification through the Mental Health and Addiction Certification Board of Oregon (MHACBO)
Demonstrated ability to communicate clearly and concisely both orally and in writing.
Previous experience with trauma informed care.
Deep understanding of and experience addressing the systemic barriers that perpetuate disparities for community members who experience marginalization.
Experience or ability to synthesize data in order to independently solve problems or formulate recommendations.

Work Location: This position works onsite at the Gladys McCoy building 619 NW 6th Ave Portland OR 97209 with meetings in Multnomah County clinics or other community sites and ad hoc remote work.
