Are you looking to expand your RN experience in the oncology field Dayton Physicians Network is looking to add an RN Infusion & Onc Navigator to our Kettering location! The Oncology Nurse Navigator is responsible for providing personalized support and guidance to patients with cancer and their families throughout the cancer care continuum. The nurse navigator will work in collaboration with the healthcare team to help patients navigate the complex healthcare system coordinate care educate the patient and ensure timely access to appropriate resources and services. The Nurse Navigator will also assist in removing any barriers to care the patient might experience. This is a shared role with infusion and the Nurse Navigator will have schedule days in that department. Schedule: Monday - Friday 8:00am to 4:30pm No evenings weekends or holidays! Essential Duties and Responsibilities: - Assist patients with cancer families and caregivers to overcome healthcare system barriers.
- Provide Educational resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer care continuum.
- Assist with managing side-effects of therapy assessing needs and matching unmet needs with appropriate services.
- Supports a smooth transition of patients from active treatment into survivorship chronic cancer management or end-of-life care.
- Participates in coordination of the plan of care with the multidisciplinary team promoting timely follow-up on treatment and supportive care recommendations.
- Acts as a liaison between the patients families and caregivers and the providers to optimize outcomes.
- Provides psychosocial support to and facilitates appropriate referrals for patients families and caregivers especially during periods of high emotional stress and anxiety.
- Facilitates communication among members of the multidisciplinary cancer care team to prevent fragmented or delayed care that could adversely affect patient outcomes.
- Facilitates shared decision making.
- Promotes advanced care planning and supports palliative care.
- Navigators will continue current role as infusion nurses on scheduled days and fulfill those requirements as indicated in the infusion nurse job description.
Required Education and/ or Experience: - Registered Nurse (RN) with a current Ohio license.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 3 years of experience in oncology nursing.
- Certification in oncology nursing (OCN) preferred.
General Skill Requirements: - Excellent oral and written communication and organizational skills
- Strong problem-solving skills.
- Working knowledge of Microsoft Office including Word Excel and Outlook.
- Experience working with an electronic medical record.
- Attendance and punctuality are essential requirements of this position to provide excellent customer service to both our internal and external stakeholders.
Physical/ Mental Demands: While performing the duties of this job the team member is regularly required to use hands to assist patients reach with hands and arms; and talk or hear. The team member frequently is required to stand walk and sit. The position requires regular lifting and/or moving up to 10 pounds and occasionally lifting and/or moving up to 50 pounds. Specific vision abilities required by this job include close vision color vision peripheral vision depth perception and the ability to adjust focus.
