We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a per diem (PRN) position at a skilled nursing facility in Gloucester MA. Key Responsibilities:

Deliver direct patient care aligned with individualized care plans
Administer medications and carry out physician-ordered treatments
Monitor and document changes in patient conditions
Support patient admissions transfers and discharges
Accurately document care in the electronic medical record (EMR) system
Collaborate with the nursing and interdisciplinary team to meet resident care goals
Maintain a clean safe and supportive environment for residents

Required Requirements:

Active and Valid LPN license in the state of Massachusetts.
CPR/BLS Certification.
Minimum 1 year of recent experience in a LTC/SNF setting.
Strong clinical and communication skills.
