Salary: Up to 59185.00 per year (depending on level of experience training and qualification)
We have an opportunity for a Senior Clinical Pharmacist to join their team of staff in thePharmacydepartment.
This is a Full time role for 37.5 hours a week. . The role holder will be required tocover a flexible shift pattern between Monday to Friday 9:00AM and 17:00PM additional Saturdays on Rota and also on call on Rota.
Duties of this role include:
Provide expert clinical pharmacy input as part of the multidisciplinary team conducting medicines reconciliation clinical screening and therapeutic drug monitoring.
Optimise medicines use through evidence-based interventions ensuring the safe and effective management of long-term and acute conditions.
Support prescribing decisions through clinical assessment risk evaluation and formulary guidance.
Lead on implementation of medicines optimisation strategies ensuring prescribing is cost-effective safe and aligned with national/local guidelines.
Contribute to the development and review of clinical guidelines protocols and policies.
Monitor and report on medicines usage compliance and safety incidents participating in root cause analysis and quality improvement.
Applicants should meet the following criteria:
GPhC-registered Pharmacist.
Significant post-registration experience in clinical pharmacy preferably in a hospital setting.
Strong clinical knowledge and experience in medicines optimisation.
Excellent communication leadership and multidisciplinary collaboration skills.
Experience in mentoring or supervising junior staff.
