Job Overview

Pulse is currently seeking a highly experienced and proactive Band 8A Locum Pharmacist to join the Integrated Care Home Service in Nottingham. This is a part-time opportunity for a pharmacist with strong clinical knowledge and experience in medicines management for older adults.

The successful candidate will work closely with a multidisciplinary team including a consultant geriatrician and specialist nurses to support the safe and effective use of medicines for care home residents. Attendance at a weekly MDT on Tuesday afternoons is essential.

Key Responsibilities

Conduct medication reviews for new care home residents following assessment by a specialist nurse practitioner.
Attend weekly multidisciplinary team meetings with a consultant geriatrician and specialist nurses.
Provide expert pharmacological input into patient care discussions and ongoing medical management.
Maintain accurate clinical documentation in line with service standards.
Apply up-to-date knowledge of medicines management for conditions common in older populations.
Contribute to service development and clinical governance initiatives.

Qualifications and Experience

Be registered with the General Pharmaceutical Council (GPhC).
Have significant experience in clinical pharmacy ideally within care home or community settings.
Demonstrate strong knowledge of pharmacology related to older adults.
Be confident in working independently and within a multidisciplinary team.
Be proactive organised and committed to delivering high-quality care.
