Role Overview: Are you the kind of Social Worker who thrives with a roomful of spinning platesand still takes the time to make everyone feel seen and heard Were looking for a Social Worker who is not only experienced and driven but also outgoing empathetic and this role you will lead our Long Term Care admissions and discharges ensuring that our residents have the best possible experience at Birkwood Village. This position is ideal for a candidate who is passionate about high-quality person-centered care in a for-profit setting. A candidate who can build strong relationships with residents families staff and our wider community. If youre someone who reads between the lines leads with heart and never backs down from a fast-paced challenge we want to meet you.
Key Responsibilities:
- Review and manage referral admissions
- Coordinate and facilitate community discharges
- Communicate effectively and solve problems
- Verify insurance details
- Possess knowledge of Medicaid Medicare and other insurance policies
- Multi-task efficiently
- Maintain a positive attitude and a desire to serve
Qualifications:
Minimum of high school diploma higher education preferred.
1-2 years of social work experience in a healthcare environment
- Preference for experience in long-term care
