Job Description

**JOB DETAILS**

**Work Setting:** Hospital/Doctors Office/Clinic

**Coverage Dates:** Start as soon as possible until a new doctor is hired.

**On-Call Schedule:** Every 6th night/weekend

**Number of Providers:** There are 4 Advanced Practice Clinicians (APCs) on the team.

**Patient Age Group:** Adults and Older Adults

**Documentation System:** EPIC (computer system for patient records)

**Required Procedures:**- Taking Electrocardiograms (EKGs)

**Qualifications Needed:**

- Must be Board Certified in Advanced Heart Failure Cardiology and have Ventricular Assist Device (VAD) certification - THIS IS VERY IMPORTANT

- Must have Advanced Cardiovascular Life Support (ACLS) certification

- THIS IS VERY IMPORTANT

- Must have completed a fellowship in Advanced Heart Failure and have experience with Left Ventricular Assist Devices (LVAD) - THIS IS VERY IMPORTANT

- Provider must live locally; only travel costs will be paid

- THIS IS VERY IMPORTANT- Must have a DEA license

- THIS IS VERY IMPORTANT- Must have an active Virginia medical license - THIS IS VERY IMPORTANT
