Position Summary
The Clinical Coordinator for Acute Inpatient Rehabilitation under the guidance of the Director of Acute Inpatient Rehab is responsible for providing clinical oversight coordination of
interdisciplinary patient care and staff support within the inpatient rehabilitation unit. The role
ensures compliance with CMS Joint Commission and state regulatory requirements promotes
evidence-based rehabilitation practices and supports the operational and quality goals of the
program in a rural hospital setting.
Essential Functions
-Provide support in patient care decision-making clinical problem-solving and complex case management.
-Monitor patient progress barriers to discharge and collaborate with the interdisciplinary team to support optimal outcomes.
-Complete pre-screen assessments for new patients.
-Complete insurance authorizations for inpatient rehabilitation admissions.
-Coordinate and support discharge planning to ensure safe transitions of care.
-Facilitate daily IDT huddles and weekly care conferences.
-Ensure development and execution of individualized care plans.
-Act as liaison between nursing therapy case management social services and physicians.
-Maintain compliance with CMS IRF Conditions of Participation Joint Commission standards
and state regulations.
-Ensure documentation accuracy and timeliness (pre-admission screening post-admission evaluation physician oversight).
-Track outcomes quality indicators and report variances to leadership.
-Participate in audits surveys and quality improvement initiatives.
-Provide mentorship education and guidance for staff related to rehabilitation standards of care.
-Identify training needs and coordinate staff development opportunities as requested by leadership.
-Collaborate with the Director on census management throughput and program growth.
-Assist with staffing coverage as needed including direct patient care when within scope of
practice.
-Monitor unit resources supplies and equipment for safe and effective care delivery.
-Will coordinate social activities for patients in IPR.
Work Experience
-Minimum of 3 years of clinical experience in acute rehabilitation acute care or related
healthcare setting.
-Knowledge of CMS IRF regulatory requirements and interdisciplinary rehabilitation model of
care.
-Demonstrated leadership communication and problem-solving skills.
Education
Current Texas license as one of the following:
-Registered Nurse (RN) with a minimum of an Associates Degree in Nursing required. Bachelors degree preferred
-Occupational Therapist (OTR/L) with a Bachelors degree required Masters or Doctorate in Occupational Therapy from an accredited program preferred.
-Physical Therapist (PT DPT) with a Bachelors degree required Masters or Doctorate in Physical Therapy from an accredited program preferred.
-Speech-Language Pathologist (SLP CCC-SLP) with a Masters or Doctorate in Speech-
Language Pathology from an accredited program preferred.
-Previous experience in care coordination or clinical coordination.
-Specialty certification (CRRN NCS PCS CBIS (or equivalent) or willingness to obtain within 2 years.
