Description

Northwest Integrated Care Clinic is a Certified Community Behavioral Health Center (CCBHC).

The Outreach Worker II will work as a member of both the CCBHC team and the patients treatment team to promote positive behavioral and physical change for patients. The Outreach Worker II is responsible for clinical oversight of patient care and to the team as it relates to the delivery of services including the assessment and service planning process and monitoring and tracking CCBHC related services. Provides and coordinates all covered services (e.g. case management outreach crisis intervention counseling in addition to the CCBHC nine core services) to address patient needs. The Outreach Worker II will be responsible for identifying locating and engaging people in treatment and integrated health services. Must be able to follow up with patients who have disengaged from services or fail to show for their intake.

Minimum Qualifications:

  • High school diploma or GED plus four (4) years experience providing behavioral health services (i.e. assessment diagnosis or treatment of an individuals behavioral health issue) in an agency to an individual or in a field related to behavioral healthor
  • Associates degree plus two (2) years experience providing behavioral health servicesor
  • Bachelors degree in a non-behavioral health related field plus one (1) year experience providing behavioral health servicesor
  • Bachelors degree in a behavioral health related field no experience necessary.

Position Schedule: Wed - Thurs 7:00am - 8:00pm; Fri 5:00am - 6:00pm; Sat 7:00am - 11:00am

Qualified Applicants:

  • Must be able to pass a pre-employment drug screen and background check.
  • Must be able to obtain a Level 1 Fingerprint Clearance Card and maintain clearance throughout employment.

Additional Requirements:

Must be 21 years of age (automobile insurance requirement) and possess a valid AZ drivers license with a clean or acceptable driving record. An unacceptable driving record includes (but is not limited to) the following:

  • more than two 2 moving violations in the last three 3 years or;
  • more than one 1 at-fault accident within the last three 3 years or;
  • any major violation (i.e. DUI Careless/Reckless Driving Drag Racing Hit and Run) within the last three 3 years.

Other driving related violations may be considered unacceptable. At time of job offer submission of 36 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.
