Job Summary
Burnett Medical Center is located in Grantsburg Wisconsin. We are just over an hour to the Twin Cities and a few miles from the St. Croix River.
We are looking for a full-time Social Worker to fill our Social Services Manager position.
SHIFT & HOURS: 1.0 FTE (40 hours/week). Days Monday through Friday with occasional evening/weekend availability needed.
RESPONSIBILITIES:
Manage Social Services department. Provide assistance and expertise in areas of patient/resident care discharge planning and home planning processes with other facility departments external organizations agencies and health care facilities. Ensure compliance quality care and regulatory standards. Other duties as assigned.

QUALIFICATIONS:
- BSW or MSW from an accredited school of social work.
- Require Social Work licensure in Wisconsin.
- Prefer experience in hospital and long term care facility.
- Must be knowledgeable of community resources and able to network within the community.
- Positive attitude strong people skills with internal and external customers strong verbal and written communication skills ability to adapt to change and to be a team player.
