Job Description

DISCLOSURE - MULTI-EMPLOYER ROLE:


This W-2 hourly Family Nurse Practitioner position includes responsibilities that span across the following five affiliated organizations all under shared leadership:


FOCUS #1 Community Wellness Outpatient Mental Health Center

FOCUS #2 Lighthouse Behavioral Health Center

FOCUS #3 Community Wellness Psychiatric Rehabilitation Program

FOCUS #4 HolBrock Estates Supportive Housing Programs

FOCUS #5 Freedom Healthcare



You will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of all three companies with hours responsibilities and compliance obligations tracked separately for each.


This role is structured as a 24-hour per week position with approximately one-fourth of the total weekly hours (about 6 hours per week) allocated to each organization. While day-to-day supervision and scheduling will be coordinated centrally to support integrated care each employer will manage its own onboarding HR and compliance processes in accordance with applicable laws and organizational policies.

DIVISION: Operations


ACCOUNTABLE TO: Operations Manager


CLASSIFICATION:

Part-time W-2 employee (24 hours per week over three scheduled days). This position is structured as a multi-program role with the following allocation of total weekly hours:

  • Freedom Health Care Freestanding General Primary Care Clinic: 15% (approximately 3.6 hours/week)
  • Community Wellness Outpatient Mental Health Center (CWOMHC): 25% (approximately 6 hours/week)
  • Lighthouse Behavioral Health Center (LHBHC): 25% (approximately 6 hours/week)
  • Community Wellness Psychiatric Rehabilitation Program (CWPRP): 20% (approximately 4.8 hours/week)
  • HolBrock Estates Supportive Housing Programs (HESHP): 15% (approximately 3.6 hours/week)


WORK SCHEDULE: Part-Time - Tuesday and Thursday - 8 AM - 5 PM / 16 hours per week


COMPENSATION RANGE:
Compensation for this position ranges from $30.00 to $90.00 per hour and is commensurate with experience expertise verified credentials and available company budget.

ANTICIPATED TRAVEL: 5% of the time. There is no additional compensation for travel.

DUTIES AND RESPONSIBILITIES:
1. Facilitate and document coordination with external primary care providers as clinically appropriate and with proper consent.
2. Prescribe medications following a full face-to-face evaluation and assess for any contraindications prior to initiating a medication regimen.
3. Educate clients and when appropriate caregivers on the purpose effects and risks of prescribed medications.
4. Monitor and adjust medication regimens as clinically indicated; conduct evaluations at a minimum every 90 days for ongoing medication management.
5. Notify the clients primary care provider of new prescriptions or medication changes every six months or as clinically required.
6. Conduct peer reviews of other prescribers work and support documentation compliance across the clinical team.
7. Participate in interdisciplinary team meetings with HR Scheduling Patient Access and other departments to support clinical and administrative needs.
8. Conduct batch documentation audits including review and signature of referrals treatment plans biopsychosocial ASAMs and intern notes.
9. Collaborate with external agencies or treatment providers in cases involving shared clients or complex care needs.
10. Accept and review incoming lab results in ICANotes; use clinical judgment to adjust care plans accordingly.
11. Ensure all documentation is completed within expected timeframes using ICANotes and is coded appropriately using evaluation and management CPT codes.
12. Contribute to quality assurance audit readiness and performance improvement initiatives to ensure compliance with COMAR CARF Medicaid and internal standards.
13. Provide support and education to clinical staff related to medication protocols and service delivery expectations.
14. Submit periodic reports and summaries related to administrative and clinical oversight duties.



PHYSICAL DEMANDS: Regularly walk stand or stoop; occasionally lift carry push pull or otherwise move objects weighing up to 25 pounds; and regularly drive a motor vehicle.

WORKING CONDITIONS: All work is performed in an office where staff is exposed to communicable diseases of patients.

COMPETENCIES AND SKILLS:
1. Provision of credentialed focused evaluations treatments and assessments to patients
2. Monitor and evaluate individual treatment plans and service modalities rendered by staff under your oversight with the goal of enhancing effectiveness streamlining implementation assuring quality and model fidelity

LEVEL OF EDUCATION / TRAINING / QUALIFICATIONS:
Candidates must meet the following minimum qualifications to be considered for this role:
Active licensure with the Maryland Board of Nursing (MBON) as a Certified Registered Nurse Practitioner (CRNP)
Masters Degree in Nursing from an accredited institution
Board certification as a Family Nurse Practitioner (FNP)
Possession of a valid DEA (Drug Enforcement Administration) registration

SUMMARY OF POSITION RESPONSIBILITIES:
To be successful in this role the employee must be a self-starter organized clinically skilled in somatic healthcare and collaborative across multidisciplinary teams. As a Family Nurse Practitioner (Somatic Evaluator) you will provide comprehensive somatic care to clients receiving outpatient mental health services. This includes medical evaluations medication management and oversight of clinical documentation and service delivery in accordance with applicable regulatory requirements.


You are authorized to bill CPT codes 99213 and 99215 for eligible somatic services line with COMAR 10.20.06 you are responsible for reviewing and documenting each clients somatic status upon program enrollment including medical history current conditions medications and recommendations for follow-up. If a client does not have a primary care provider you will ensure referral plans are developed and executed with proper consent and collaboration maintained between the treatment coordinator and the primary care provider.


Additionally under COMAR 10.21.20.08 you will support the integration of co-occurring substance use disorder treatment into mental health care plans or coordinate with certified substance use providers when necessary. As part of the clinical team you will also ensure that staff are supported in understanding medication safety and monitoring protocols and that all medication servicesincluding prescribing administration education and documentationare delivered with the highest standards of care.

SUMMARY OF POSITION RESPONSIBILITIES:
The Family Nurse Practitioner (FNP) at Lighthouse Behavioral Health Center serves a critical role in delivering Medication Assisted Treatment (MAT) services for individuals with opioid use disorder and other substance use conditions. The FNP will provide evidence-based person-centered care under the guidelines of COMAR 10.09.80.05 and is authorized to bill CPT codes 99202 HG 99203 HG 99204 HG 99205 HG 99211 HG 99212 HG 99213 HG and 99215 HG for MAT services.


The FNP will be responsible for conducting clinical assessments prescribing and managing buprenorphine and other FDA-approved medications monitoring clients for safety and effectiveness of treatment and working collaboratively with behavioral health professionals to ensure integrated wraparound care. This role requires knowledge of pharmacological treatment protocols withdrawal management flexible tapering strategies and the ability to evaluate each clients individual dosing needs.


In accordance with COMAR 10.09.06.04 the FNP must be available on-site and on-call each week to support medical service needs across all residential treatment programs operated by Lighthouse Behavioral Health Center.


The FNP also contributes to regulatory compliance treatment planning documentation and coordination with internal staff and external care providers. SCHEDULED DUTIES AND RESPONSIBILITIES:
Be available on-site and on-call each week to fulfill medical oversight and urgent care needs at each of Lighthouses residential treatment program sites in accordance with COMAR 10.09.06.04.
Conduct initial and ongoing face-to-face medical assessments of clients seeking or receiving Medication Assisted Treatment.
Prescribe and manage buprenorphine and other FDA-approved medications in compliance with all applicable state and federal regulations.
Evaluate clients for contraindications to medications explain risks and benefits and support shared decision-making.
Monitor clients withdrawal symptoms and response to medication and make dosage adjustments as clinically indicated.
Support client-requested flexible tapering and withdrawal management ensuring safety and dignity in all treatment adjustments.

Provide medication education to clients and authorized caregivers including storage usage expected effects and side effects.
Notify with proper consent any individuals responsible for medication administration or oversight of medication changes or new regimens.
Maintain timely accurate clinical documentation in the EHR using appropriate HG-modified CPT codes.
Collaborate with the interdisciplinary team including counselors peers and case managers to ensure continuity of care and recovery support.
Participate in internal case reviews team meetings and quality improvement or audit activities.
Adhere to all standards outlined by COMAR 10.09.80.05 COMAR 10.09.06.04 DEA regulations and internal policy guidelines.

The Nurse Practitioner Consultant plays a critical advisory and clinical oversight role within Community Wellnesss Psychiatric Rehabilitation Program (PRP) operating under the Maryland Department of Health (MDH) Health Home model. This position is responsible for providing medical consultation and clinical support to ensure integrated care for individuals with serious mental illness and co-occurring chronic health conditions.


In accordance with the MDH Health Home Manual the Nurse Practitioner Consultant must dedicate a minimum of 1.5 hours per Health Home enrollee per 12 months focusing on assessment review case consultation care planning and quality improvement efforts. The provider must meet the requirements set forth in COMAR 10.27.07 and serve as a clinical leader within the PRPs multidisciplinary Health Home team.

Be available on-call to provide the below duties in the unlikely event the two primary delegating nurses are unavailable.
The Delegating Nurse Case Manager is responsible for overseeing clinical care regulatory compliance and safe nursing practice within the assisted living program.
