Scope of Service: Entitlements Case Manager Independent Contractor
CLASSIFICATION: Part-Time Contract Position (1099). Fee-for-service.

DISCLOSURE MULTI-ENTITY CONTRACTOR ROLE:
This 1099 Independent Contractor role for a Entitlements Case Manager includes responsibilities that span across the following two organizations all operating under shared leadership:
FOCUS #1: Lighthouse Behavioral Health Center LLC
FOCUS #2: Community Wellness Outpatient Mental Health Center LLC
FOCUS #3: Community Wellness Psychiatric Rehabilitation Program
As an independent contractor you will maintain separate contractual agreements with each organization. You will invoice and receive compensation separately from each entity based on the time worked and services delivered for their respective programs. This means you are not an employee of any of the entities and are instead engaged as a self-employed contractor for each.
Though the contactor is paid on a fee for service basis and not an hourly basis the services will require approximately 40 hours per week in to render assigned services as scheduled over 5 days with an estimated one-half of the services rendered allocated to each organization. While day-to-day scheduling and care coordination may be managed collaboratively to support continuity across programs each organization retains responsibility for its own scope of work compliance requirements and payment processes.
As a 1099 contractor you are responsible for your own taxes benefits and liability coverage and you must adhere to all contractual terms and professional licensing requirements relevant to each organization.
Overview
Generally assigned clients will reside in community based group homes. Assigned clients who do not reside in group homes may receive services in your assigned community based office at the companys main office or via telehealth if you or the client are in one of these locations. We are seeking a qualified and licensed Entitlements Case Manager to provide case management services on an independent contractor basis. The contractor will schedule and facilitate group and individual case management sessions conduct case management assessments and complete client documentation within the designated service period.

COMPENSATION RANGE: Ranges between $50000 per year to $100000 per year paid on a fee-for-service basis for servicing assigned clients as scheduled.

GENERAL DUTIES AND RESPONSIBILITIES:
Assist supervisor with work-related requests.
Collaborate with and serve as program POC for external treatment providers and
outside agencies as needed.
Responsible for following regulations of COMAR CARF any other regulatory body and company policies/ procedures related to service delivery and documentation and co-facilitating orientation of all new hires to these as well.
Support the maintenance of a safe environment by participating in training and drills

The Community Wellness Psychiatric Rehabilitation Program (PRP) is a core service of Community Wellness Outpatient Mental Health Center designed to support individuals living with serious mental illness in achieving stability
independence and improved quality of life. Our PRP provides structured
community-based support focused on developing life skills managing symptoms and promoting personal recovery goals.
Through individualized service planning case management health home coordination and peer support the PRP helps clients navigate housing
employment education and health care access. Services are person-centered trauma-informed and culturally responsive with an emphasis on whole-person
care and long-term wellness. The program serves both youth and adults working in close collaboration with clinical providers families and community partners.

A) Health Home Care Manager (HHCM) W1760 / W1761
Coordinate medical behavioral and community services; track screenings/labs/hospital encounters (e.g. via CRISP).
Provide health education; monitor prevention/wellness initiatives and care-plan goals.
Document timely in ICANotes and meet W1760/W1761 billing rules and audit standards.
B) Psychiatric Rehabilitation Specialist (PRS) H0002
Complete Rehab Assessments; develop/review IRPs/ITRPs; oversee skills and recovery programming.
Coach direct care staff on plan fidelity; document to H0002 billing and COMAR standards.
Perform only tasks within your license/scope of practice.
Key Responsibilities
Step in as backup coverage upon CWPRP request.
Meet documentation timelines and quality requirements.
Support audit readiness and QI efforts; uphold HIPAA and confidentiality.
Collaborate with CWPRP staff and external partners as needed.

Qualifications
Education: Bachelors in psych/social work/counseling/nursing or related (Masters preferred).
Licensure/Certification (one or more): LCADC LGADC CAC-AD CSCAD ADT LCPC LGPC LCSW-C LCSW LMSW LBSW LCMFT LGMFT Licensed Psychologist Registered Psychology Associate CRNP-PMH APRN-PMH CNS-PMH or eligible student intern.

Insurance Requirements (Contractor-Carried)
Contractor must maintain at Contractors expense the following coverages and furnish Certificates of Insurance (COIs) before activation and upon renewal:
1. Professional Liability (Malpractice): Minimum $per claim / $aggregate (claims-made acceptable). If claims-made provide 2-year tail coverage post-engagement or continuous prior-acts coverage.
2. General Liability: Minimum $per occurrence / $aggregate.
3. Automobile Liability: If driving on assignment at least $300000 combined single limit (or state minimums if higher).
4. Cyber / Privacy Liability: Recommended $250000 for PHI/PII incidents (may be required for remote/EHR access).
5. Workers Compensation/Disability: As required by law for Contractors own business.
COI Requirements:
Certificate Holder: Community Wellness Psychiatric Rehabilitation Program (CWPRP).
Additional Insured: CWPRP on GL (and Auto if applicable) with Primary & Non-Contributory wording and Waiver of Subrogation where available.
Notice of Cancellation: 30 days notice requested.
CWPRP may suspend assignments for lapsed insurance.
Invoicing & Payment Requirements
A) Eligibility to Invoice
Invoice only for authorized assignments actually performed and fully documented in ICANotes (signed/locked).
No billing for admin time travel or training unless pre-authorized in writing.
B) Invoice Contents (required)
Include a clean legible invoice with:
Contractor legal name/entity remit address email/phone and EIN/SSN (W-9 on file).
Unique invoice # and invoice date.
Service dates and total hours/units by billing code (W1760 W1761 H0002).
Hourly rate and line-item extensions; total amount due.
Assignment/location (site or telehealth) and client identifiers using non-PHI internal IDs (no names/DOB/SSN).
Reference to ICANotes encounter IDs or attached time/units export (if requested).
Attestation: I attest services were rendered as authorized documented in ICANotes and comply with CWPRP standards and applicable regulations.
C) Submission & Timing
Cadence: Weekly or bi-weekly as directed during onboarding (submit within 5 business days after period end).

D) Disputes Corrections and Holds

You will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of all two companies with service documents responsibilities and compliance obligations tracked separately for each.
Though the employee is paid on a fee for service basis and not an hourly basis the services will require approximately 40 hours per week in to render assigned services as scheduled over 5 days with an estimated one-half of the services rendered allocated to each organization. Our PRP provides structured community-based support focused on developing life skills managing symptoms and promoting personal recovery goals.
Through individualized service planning case management health home coordination and peer support the PRP helps clients navigate housing employment education and health care access. Services are person-centered trauma-informed and culturally responsive with an emphasis on whole-person care and long-term wellness. The program serves both youth and adults working in close collaboration with clinical providers families and community partners.

Community Wellness Psychiatric Rehabilitation Program (CWPRP):

WORK SCHEDULE: Services may be scheduled during regular business hours Monday through Friday 8:00 AM 5:00 PM
COMPENSATION RANGE: Ranges between $30 - $45 fee-for-service commensurate with experience and are paid via biweekly payroll subject to standard withholdings.

ANTICIPATED TRAVEL: Up to 10% of the time (interoffice and site-based)
GENERAL DUTIES AND RESPONSIBILITIES:
Provide backup coverage for primary PRS/HHCM staff as requested.
Document all services accurately and within required timeframes (2448 hours).
Participate in interdisciplinary meetings to support client care and program operations.
Support audit readiness quality improvement efforts and compliance initiatives.
Maintain professional conduct consistent with CWPRP policies HIPAA and confidentiality standards.
PHYSICAL DEMANDS:
Regularly required to walk stand or stoop.
Occasionally required to lift up to 25 pounds.
May need to travel locally to provider offices or community agencies.

SUMMARY OF POSITION RESPONSIBILITIES:
The Backup Psychiatric Rehabilitation Specialist / Health Home Care Manager (PRS/HHCM) provides as-needed coverage to ensure the Community Wellness Psychiatric Rehabilitation Program (CWPRP) maintains compliance with COMAR Medicaid and accreditation standards. This position is activated only at the request of the company typically to cover vacancies absences or times of increased demand. The role supports both rehabilitation services and integrated health coordination for clients with serious mental illness and co-occurring conditions.
SCHEDULED DUTIES & RESPONSIBILITIES:
Essential Duties and Responsibilities
Health Home Care Manager (HHCM) W1760/W1761
Coordinate medical behavioral and community-based care for assigned participants.
Track and follow up on screenings labs and hospital admissions/discharges.
Provide health education to clients and caregivers on chronic disease and wellness topics.
Maintain documentation in ICANotes in compliance with billing and audit standards.
Psychiatric Rehabilitation Specialist (PRS) H0002
Conduct Rehabilitation Assessments and develop or review Individual Rehabilitation Plans (IRPs) and Individual Treatment and Rehabilitation Plans (ITRPs).
Oversee life skills development rehabilitation programming and recovery support.
Train and support direct care staff in implementing client-specific rehabilitation goals.
Ensure compliance with COMAR 10.21.21.10 and Medicaid H0002 billing standards.
Experience
Minimum 2 years of experience in behavioral health psychiatric rehabilitation Health Home coordination or community-based case management.
Knowledge Skills and Abilities
Familiarity with ICANotes and CRISP or other EHR/HIE platforms.
Strong documentation and organizational skills.
Ability to work with diverse and vulnerable populations.
Trauma-informed and culturally responsive practice.
Knowledge of Medicaid COMAR and CARF standards.

Education:
Bachelors degree in psychology social work counseling nursing or a related field required.
Masters degree preferred.
