Headquartered in Santa Monica CA CLAREMATRIX maintains 7 facilities in the Southern California region; providing services to participants in a manner consistent with its Core Values: Compassion Teamwork Integrity Empowerment Adaptability and Equality.
Primary Objectives:
The Care Coordinator works with supporting staff on assigned grants and programs within the organization. The Care Coordinator is responsible for using a collaborative and coordinated approach to linking participants with appropriate health and social services to address their specific needs and treatment goals. This role requires that the Care Coordinator is knowledgeable of relevant community resources and effective at communicating with other professionals providers and participants to successfully link participants to services. The role of the Care Coordinator is to offer support services to participants to increase self-efficacy self-advocacy basic life skills coping strategies self-management of biopsychosocial needs benefits and resources and reintegration into the community. This is a full-time position that is 100% devoted to care coordination and case management for grant participants.

All employees whose functions require or necessitate contact with clients or food preparation shall complete a health screening report or a health questionnaire. Health screening reports or health will complete at minimum a self-report screening questionnaire.
TBtestistobeconductedunderlicensed medicalsupervisionnotmorethan60calendardaysprior to or 7 calendar days after employment and renewed annually from the date of the last tuberculosis with a known record of tuberculosis or record of positive testing shall not be required to obtain a tuberculosis skin test. Unless there is documentation that the staff completed at least 6 months of preventive therapy the staff shall be required to obtain within 45 calendar days of employment a chest x-ray result and a physicians statement that the incumbent does not have x-ray within the prior 6 months is acceptable. The physicians statement shall be renewed annually.
