Under the direction of the LPN Restorative Nurse Coordinator youll implement individualized care plans and ensure compliance with federal state and local regulations.
Key Responsibilities:
- Deliver Level A/B restorative care (e.g. therapeutic exercises ambulation transfers ROM dining assistance).
- Document care and resident progress.
- Collaborate with nursing staff and assist in training CNAs.
- Support restorative dining and restraint reduction programs.
- Foster a caring respectful environment aligned with Fellowships Mission and RISE Core Values.
Qualifications:
- Current PA CNA certification with 2 years clinical experience.
- Basic Life Support certified.
- Strong communication and computer skills.
- Knowledge of geriatric and dementia care.
- Must meet Pre-Hire Assessment requirements.
- Fully vaccinated against influenza and COVID-19 (medical/religious exemptions considered).
