Job Summary
The Admissions Counselor plays a pivotal role in guiding prospective patients and families through the admission process at MeadowWood Behavioral Health System. This position requires a compassionate and detail-oriented individual who can assess patient needs provide clinical support and ensure a smooth transition into care. The Admissions Counselor works closely with patients families and referral sources to evaluate needs coordinate treatment plans and promote MeadowWoods programs and services.
Responsibilities
- Conduct individual counseling sessions as well as group or family sessions and document appropriately.
- Complete initial and follow-up assessments to determine patient needs and appropriate level of care.
- Facilitate group or family sessions and support patients in modifying problem behaviors.
- Prepare individualized treatment plans and coordinate case management duties to ensure quality care.
- Provide counseling and support services to families of patients.
- Serve as a liaison between referral sources patients and internal care teams.
- Deliver crisis intervention when needed.
- Develop aftercare planning for patients to support successful transitions post-treatment.
- Maintain timely accurate and concise documentation of care treatment and incidents.
- Assist with training new staff and interns and provide guidance to clinical or direct patient care personnel as assigned.
Education & Experience Requirements
- Masters degree preferredin social work psychology counseling or a related field.
- Bachelors degree in a social or health services field with 6 years of counseling experience; OR
- Bachelors degree in another field with 8 years of counseling experience; OR
- Masters degree in a social or health services field with 4 years of counseling experience.
- Degree must be from an accredited college or university.
