What Youll Do

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
• Create and manage personalized care plans for ongoing conditions
• Collaborate with our integrated care team (nurse case managers NPs CHWs)
• Ensure thorough documentation in our EHR and compliance with quality measures
• Engage patients in preventive care to reduce avoidable complications

What Were Looking For

• MD or DO Board Certified/Eligible in Family Medicine Internal Medicine or Geriatrics
• Active medical license multi-state licensure a plus
• Credentialed with major plans: UHC Aetna Humana
• Strong interest in preventive care senior health or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
• Collaborative mission-driven mindset with excellent communication skills
