Summary:
The Osborne Grove Assessment Centre provides 37 beds of off-street accommodation for rough sleepers with complex needs. As an Assessment and Move On Worker you will be responsible for undertaking detailed assessments of individuals accessing the service developing tailored move-on plans and supporting clients to secure appropriate longer-term housing options within 12 weeks. The role involves close collaboration with service users to address their individual needs promote harm reduction in relation to health and substance use and ensure that safety plans ID and benefits are in place. You will play a key role in supporting individuals with multiple disadvantages including those with No Recourse to Public Funds (NRPF) while working in a supportive and flexible team environment on a rolling rota basis.
Responsibilities:
- Conduct thorough needs and risk assessments for all service users on entry to the service.
- Provide practical and emotional support to service users with complex needs including those with NRPF.
- Build strong professional relationships with statutory agencies voluntary organisations and housing providers to facilitate positive move-on outcomes.
- Uphold safeguarding responsibilities by identifying and responding to risks and concerns appropriately
Essentials:
- Educated to Level 3 (NVQ or equivalent) in Health & Social Care Housing or a related field
- Demonstrable experience of working with vulnerable adults rough sleepers or people with complex needs.
- Understanding of safeguarding adults and children procedures.

- This role requires an Enhanced DBS
