Community Day Service Manager - Elgin

$1000 Sign on Bonus

The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical developmental intellectual mental health challengesthose who have suffered a trauma and those at risk to enjoy lives of dignity and purpose. We are looking for aCommunity Day Service Managerwho exemplifies that mission and wants to work for an organization that makes a difference.

Be available to work Monday- Friday from 8:00am to 4:00pm.

What will you be doing

Responsible for day-to-day operations of the Community Day Services program and for assuring compliance with all applicable DHS DPH CARF and local rules and standards and agency policies and procedures. Supervise assigned staff and provide clients and staff members with a team oriented positive professional and safe environment to learn and safety staffing and client documentation in accordance with all applicable state local and agency standards and all applicable documentation records and reports related to provided services and supports and functions as a member of the Interdisciplinary input and information to the IDT as needed and functions as a QIDP if assigned developing and implementing individual programs and maintaining documentation and records as required.


Job Responsibilities:

Program Responsibilities

  • Assure program compliance to all rules regulations and recommendations per program state and federal guidelines including AID procedures and applicable DHS IDPH CARF HCBS and Medicaid Waiver/CMS rules
  • Provide input on program philosophies policies and procedures in accordance with current knowledge and expertise in the field
  • Ensure the provision of appropriate functional and stimulating programming for clients in activities of daily living health promotion work and participation in the community
  • Maintain program materials and update as needed based upon client needs/abilities
  • Create and maintain all required electronic and/or hard copies of program records such as personnel information client information communication logs external and internal monitoring reports and audits safety documentation and staff/client/committee meeting minutes
  • Submit and assure timely and accurate paperwork for billing incident/accident reports and client input meetings and staff meetings
  • Review and assist to update program procedures as requested

Complete following trainings:

o DSP within 120 days of hire (if needed)

o QIDP Orientation Curriculum within 6 months of hire

o Maintain current certification in CPR and FA

o Maintain current certification in CPI

o OIG Rule 50 annually

o HIPAA Infection Control and Hazard Communication annually

o Other trainings as assigned

o Complete QIDP 12 CEUs annually following QIDP training

  • Monitor and assist program to ensure compliance with financial and HR policies and procedures regarding purchases and budgets
  • Assist to oversee maintenance repair inspections and corrections of facility/grounds/vehicle and housekeeping to assure a safe environment
  • Work to continuously improve relationships with AID stakeholders referral sources and providers of support services

Staff/Supervising Responsibilities

  • Supervise DSPs and CDS Instructors to assure client programming documentation safety rights confidentiality and compliance with agency state and federal regulations
  • Facilitate teamwork and positive interactions & attitudes among staff
  • Recruit interview and hire staff according to AID policies and procedures including seeking input from staff and clients
  • Orient all staff to job program and agency and provide on-going training per procedures
  • Develop coverage schedules input schedules into When 2 Work program and supervise and monitor staff schedules ensuring compliance with relevant labor laws and AID procedures
  • Complete staff performance evaluations at 60 days after hire per AID procedures
  • Monitor and approve staff time cards and PTO on a weekly basis through payroll management system (Paycom)
  • Provide regular communication to staff and facilitate effective meetings and in-service trainings as required
  • Ensure staff and clients are trained in safety requirements and AID procedures regarding safety.

Responsibility to Clients

  • Manage a caseload of clients per AID and regulatory standards if assigned
  • If assigned a caseload organize and conduct annual planning meetings and other meetings for clients as necessary to meet agency and program rules and regulations
  • Provide training guidance and monitoring to staff to assure proper implementation of all clients Implementation Strategies and/or Behavior Support strategies as needed
  • Develop and provide instruction/training to clients as defined in the clients Implementation Strategy or Individual Plan to enhance and build skills including in areas of health promotion and community integration
  • Actively support the clients needs for adaptive equipment and assistive technology through assessment seeking resources and implementation

Additional Job Responsibilities:

  • Represent CDS programs or agency on internal and external committees as requested
  • Organize prioritize and respond with flexibility and objectivity to meet the requirements of the job and the challenges of growth
  • Perform other job-related tasks as assigned

What will you bring to the table

Education

  • Bachelors degree in a human service field required; Masters Degree preferred

License/Certifications

  • Must meet qualifications as a QIDP (at least one-year experience with people with developmental disabilities and a bachelors degree in related field (See DHS QIDP Checklist A for full list of acceptable degrees)
  • Attend and pass DHS mandated QIDP training within 6 months of hire; maintain QIDP CEUs

Experience:

  • A minimum of two years paid experience in the field of developmental disabilities required
  • A minimum of 1 year of supervisory experience required

Additional Requirements:

Physical:

  • Requires significant walking and standing daily with occasional running for short periods of time
  • Must be able to push pull reach stoop and bend
  • Must be able to lift or move up to 50 lbs with no physical or medical restrictions
  • Must be able to assist individuals with their physical needs and personal care including a two-person lift or transfer with no physical or medical restrictions
  • Must be able to intervene physically using applied CPI techniques and/or restraints after training with no physical or medical restrictions

Equipment:

  • Using a phone and voicemail system walkie talkie or intercom system when assigned
  • Computer experience with working knowledge of Microsoft Windows applications: Word Excel and PowerPoint.
  • Alarm Systems (weather fire security etc.) where present
  • Office Equipment (printer copy machine calculator shredder fax.)
  • Audio/visual equipment (LCD projector overhead projector DVD Smartboard)
  • Adaptive equipment (i.e.: Hoyer lift communication devices other assistive devices scales - electric & manual) wheel chairs and wheel chair lifts in vehicles
  • Household appliances (stove microwave washer and dryer vacuum etc.)

Additional Requirements:

  • Minimum age requirement is 18 years old
  • Valid unexpired Illinois drivers license and good driving record as defined by AIDs driver eligibility requirements
  • Minimum liability insurance as defined by AIDs policy
  • Ability to transport clients in agency and/or personal vehicle as needed to support programming
  • Complete DHS-approved Direct Support Person (DSP) training within 120 days of hire OR have already completed DSP training and appears on the Health Care Worker Registry
  • Complete and maintain assigned trainings and certifications as required by program location including but not limited to: CPR/FA CPI Positive Behavior Supports AIDs Health Curriculum and others as assigned
  • Ability to complete job requirements with minimal supervision
  • Ability to provide crisis intervention when needed and work as part of a team to support individuals with complex needs
  • Attend or lead in-service trainings as required
  • Attend AID Managers Meetings as required
  • Demonstrate effective written and oral communication skills
  • Demonstrate proficient computer skills
  • Good interpersonal skills and maintain professional interaction with co-workers and supervisory staff
