About the Role
The Planned Surgical Liaison Nurse will co-ordinate negotiate and manage the transition of care of patients throughout the continuum. You will be primarily responsible for managing the Planned Surgery preparation list according to the access policy streamline the elective admission process and work towards decreasing wait for surgery and length of stay.
Aware of the Planned Surgery Access Policy (2024) principles with regards to when patients are placed on the preparation list
Liaise with surgical staff and other relevant stakeholders to ensure appropriate case management of patients
Monitor elective surgery cancellations and arrange appropriate follow up
Monitor Category 1 patients to ensure they are treated within 30 days.
Manage Category 2 & 3 patients to ensure they are treated within their designated time frame set by the Department of Health
Ensure auditing is up to date and patient care coordination facilitated in accordance with the planned surgery access policy (2024).
Classifications for this position will range from YU11 ($102278.80 per annum pro rata)
Your Contribution
Bachelor of Nursing which meets the registration requirements of the Nursing and Midwifery Board of Australia.
At least 3 years postgraduate acute surgical nursing experience
Current National criminal history check or willing to obtain

About the Planned Surgery Team
The Planned Surgery Management Team coordinates and case manage patients that are on the St Vincents Hospital preparation list in accordance with the Planned Surgery policy and the Department of Health Access Policy. The ESMT is responsible for the case management all patients across the surgical specialties at SVHM.
