Our role is also supporting people to self-manage long-term health conditions such as diabetes and mental health challenges with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people including older people and people with disability to stay living at home longer and to live well in their community.

Our broad range of primary health services care management and in home and community support services can be tailored to an individuals needs and with major hubs in Box Hill and Eltham these services can be delivered locally in the Eastern and North Eastern suburbs of Melbourne.

Description

The opportunity:
We are looking for a Peer Support Worker to join our supportive and professional multidisciplinary teams in on a fixed term contract working part time (0.8FTE).

Responsibilities include but are not limited to;

  • Intentionally and safely share your lived experience of recovery.
  • Provide a connection to care and social supports.
  • Provide targeted follow up andoutreach to people during their recovery journey bringing a lived experience/peer understanding to the relationship.
  • Engage with individuals community groups and clients who are risk of/have mental health issues.
  • Support clients through intake and assessment processes and refer to other services as needed.
  • Work as part of a multidisciplinary team within a supportive inclusive and diverse workplace.


Skills And Experiences

Our ideal candidate:

  • Lived experience of recovery from mental health issues.
  • Experience working in a mental health setting delivering intentional peer support services for people experiencing mental health issues.
  • Able to demonstrate empathy particularly in the development of the client-peer worker relationship.
  • Ability to advocate appropriately for clients the profession and healthAbility.
  • Understanding of principles of clinical risk within a community health and mental health services context
  • Demonstrated ability to work independently and in a team environment
  • Relevant training/qualifications in peer support (such as a Cert IV in Peer Support or accredited Intentional Peer Support Program)
  • Strong verbal and written communication skills and computer literacy including data entry
