Job Summary
Primary Care Clinic at Tsehootsooi Medical : Care Coordinator (case manager) nurse positions within our Primary Care clinic:Requirement: Three (3) year Case Management : Weekdays Monday through Fridays from 8a to 5p with One (1) hour lunch no weekends or holidays. Collaborate with support nursing staff and with providers for patient : Information can be found under the Facility Group Document section. Orientation: on Mondays only - If your traveler misses the compliance deadline they will be pushed to the following scheduled Orientation.
Shift: M-F 8-5/730-430 no holidays no weekends. Maximum of 2 Days Call off (Unapproved RTO) during contract.
