Our client a trusted healthcare consulting and hospital management partner is seeking a Licensed Clinical Social Worker (LCSW) to join a hospital-based Rural Health Clinic in Worland WY. This is a full-time permanent position working onsite with a supportive team and strong leadership.
Position Overview:
The Licensed Clinical Social Worker (LCSW) is an essential member of the primary care team providing assessment intervention and care coordination for patients with both medical and behavioral health needs. This role includes delivering evidence-based therapy conducting screenings supporting chronic disease management and connecting patients with community resources. The LCSW will also collaborate with care managers and community health workers to address unmet social needs and improve overall patient outcomes.

Compensation: $65000 $80000

Responsibilities:
- Conduct patient interviews and assessments; assist in developing care plans
- Provide screening assessment and treatment for behavioral health conditions (depression anxiety substance use etc.)
- Utilize common screening tools (PHQ-9 GAD-7 SBIRT etc.) and evidence-based practices
- Support team-based care for patients with chronic health conditions (e.g. diabetes heart disease hypertension obesity)
- Deliver psychotherapy services for individuals and families; perform psychiatric diagnostic evaluations
- Collaborate with interdisciplinary teams to enhance patient care outcomes
- Document patient encounters in the electronic health record (EHR) in a timely and accurate manner
- Provide culturally sensitive care and adapt services to meet language and diversity needs
- Support use of telehealth technologies and patient education on digital tools
- Monitor health outcomes at both patient and population levels
- Identify and connect patients to appropriate community resources and support services
Qualifications:
Required:
- Masters degree in social work (MSW) from an accredited institution
- Licensed Clinical Social Worker (LCSW) in Wyoming active and in good standing
- Minimum of 2 years of clinical social work experience
Preferred:
- Experience in medical FQHC or rural health settings
- Prior work with diverse patient populations
- Familiarity with integrated behavioral health models
Skills & Abilities:
- Strong advocacy skills with a patient-first mindset
- Professional communication and ability to navigate conflict with tact
- Creative problem-solving skills including outside the healthcare setting
- Ability to build relationships with community organizations and resources
- Strong time management skills; ability to work independently in dynamic environments
- Proficiency with EHR systems care coordination platforms and telehealth tools
