Job Summary
We are seeking a visionary leader to join our Philadelphia team. This role elevates the standard of patient care within home health guiding the team to maintain the highest standards of healthcare in a patient-centered manner.
Key Responsibilities:
- Oversee and direct patient care services ensuring compliance with regulations.
- Supervise mentor and support nursing staff to enhance patient outcomes.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Maintain accurate and timely documentation of patient care.
- Conduct regular audits for quality assurance.
- Act as liaison between patients families and healthcare providers.
- Participate in quality improvement and professional development initiatives.
Qualifications & Requirements:
- Minimum two years of management or supervisory experience in a home care setting preferred.
- Familiarity with professional standards Medicare Conditions of Participation and regulatory requirements.
- Current RN license in Pennsylvania.
