Job Description

Summary:
-
Provide social work services for geriatric participants in a PACE/Adult Day setting; primarily center-based with moderate/light home assessments; develop integrated care plans coordinate community/financial resources and ensure continuity across settings.

Responsibilities:
-
Perform comprehensive psychosocial assessments/reassessments
-
Provide one-on-one and group counseling
-
Collaborate with the interdisciplinary team to create person-centered care plans
-
Document timely and accurately in EMR (Epic preferred)
-
Coordinate community benefits/resources (Medicare/Medicaid/WA Apple Health)
-
Conduct home visits within the service radius; self-schedule within 8am 5pm
-
Adhere to CMS/DOH/Joint Commission standards
-

Key Requirements:
-
2 years post-graduate social work experience with geriatric population
-
MSW required
-
BLS required
-
One active WA social work license: LICSW LSWAIC or LASW
-
EMR proficiency (Epic preferred)
-

cultural competence

Valid driver s license and reliable transportation

Qualifications
-
Education: MSW required
-
Experience: Minimum 2 years with geriatric/older adult populations (PACE/Adult Day LTC/SNF community health or hospital CM acceptable)

License: Active WA LICSW or WA LSWAIC or WA LASW (one required; temporary acceptable); BLS required
-
Skills: Psychosocial assessment; motivational interviewing; crisis intervention; interdisciplinary care planning; community/benefits navigation; EMR/Epic; time management; autonomous scheduling; safe driving
