Job Summary:
This part-time (24 hours/week) temporary-to-permanent Licensed Social Worker position supports an adult day care program. The social worker provides direct social work services to participants conducts comprehensive assessments accesses community resources and ensures continuity of care. A portion of the work involves home visits within Snohomish County with mileage reimbursement for travel between work locations.

The social worker provides direct social work services to participants conducts comprehensive assessments accesses community resources and ensures continuity of care. A portion of the work involves home visits within Snohomish County with mileage reimbursement for travel between work locations.
Location: Everett Washington United States
Responsibilities:
- Provide direct social work services to participants in an adult day care program.
- Conduct one-on-one and group counseling.
- Collaborate with interdisciplinary team members.
- Conduct comprehensive social emotional cognitive and ethnic assessments.
- Develop individualized integrated plans of care.
- Access community and financial resources for participants.
- Ensure continuity of care across the continuum.
- Conduct home visits as needed to conduct assessments.
- Maintain accurate documentation.
- Travel within Snohomish County for home visits (mileage reimbursed).
- Conduct one-on-one and group counseling.

Required Skills & Certifications:
- Masters degree in Social Work (MSW).
- Minimum two years of experience working with a geriatric population.
- Active WA LICSW WA LSWAIC or WA LASW license (temporary licensure accepted).
- BLS certification.
- Minimum two years of experience working with a geriatric population.
- Active WA LICSW WA LSWAIC or WA LASW license (temporary licensure accepted).
- BLS certification.

Special Considerations:
- Temporary-to-permanent position (13-week contract).
- Part-time (24 hours/week).
- Home visits required (within Snohomish County).
- Mileage reimbursement for travel between work locations.
- Work hours are expected to be within the 8am-5pm workday.
- Open to both local candidates and travelers including first-time travelers.
- RTO restrictions to be confirmed with the hiring manager.
