Job Summary
Shift and Status
Full time Monday to Friday 11:00 p.m. to 9:30 a.m.
Every other Weekend 8:00 p.m. to 8:00 a.m.
Hybrid: Out in the field visits with patients in three counties Van Buren Cass and Berrien.

Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less serving individuals in their home nursing facilities and in the hospital. About Hospice
In collaboration with the entire interdisciplinary team assess plan implement coordinate monitor and evaluate the strategies and services required to meet a patient/familys end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free peaceful sacred death.
Scope of work
Under the direct supervision of the Nursing Manager and in collaboration with the interdisciplinary team the Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors detects plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.
Qualifications
Required
- Associates degree in nursing
- 2 years of relevant experience
- Michigan Registered Nurse (RN) license upon hire
- State of MI Drivers License upon hire
Preferred
- Bachelors degree in nursing
- 1 year of relevant experience of acute care emergency or rehab facility experience for all of the Home Care business lines
