  • Status:Full Time and Part-Time opportunities available (Part time Minimum commitment of 5 visits per week)
  • Compensation:$75-$150 per visit PLUS travel and mileage compensation

As a Home Health Physical Therapist heres a big-picture view of what youll do:

  • Provide home-based physical therapy to patients who have lost mobility strength and functionality due to an illness or injury
  • Work as part of the health team which may include RN LPN CNA HHA OT and SLP focused on the patients plan of care and goals
  • Observe movements conduct assessments document progress and modify care plan as needed
  • Assist patient with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients on their plan of care established goals exercises and self-care
  • Coach family on the plan of care therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:

  • Graduate of an accredited Physical Therapist Program and active PT license in Colorado
  • Minimum 1 yearof physical therapy experience ideally in home healthcare
  • BLS/CPR certification(demonstration course required)
  • Knowledge of state and federal home health regulations
  • Good clinical judgment strong interpersonal skills dexterity and compassion
  • Pass a background check and drug test (we are required to test for THC)
  • No Covid Vaccination Required
