- Location: Fort Collins and surrounding area
- Status:Full Time and Part-Time opportunities available (Part time Minimum commitment of 5 visits per week)
- Compensation:$75-$150 per visit PLUS travel and mileage compensation

As a Home Health Physical Therapist heres a big-picture view of what youll do:
- Provide home-based physical therapy to patients who have lost mobility strength and functionality due to an illness or injury
- Work as part of the health team which may include RN LPN CNA HHA OT and SLP focused on the patients plan of care and goals
- Observe movements conduct assessments document progress and modify care plan as needed
- Assist patient with exercises and hands-on therapy to manage pain and increase mobility
- Educate patients on their plan of care established goals exercises and self-care
- Coach family on the plan of care therapy goals and safely assisting with exercises
A few must-haves for Home Health Physical Therapists:
- Graduate of an accredited Physical Therapist Program and active PT license in Colorado
- Minimum 1 yearof physical therapy experience ideally in home healthcare
- BLS/CPR certification(demonstration course required)
- Knowledge of state and federal home health regulations
- Good clinical judgment strong interpersonal skills dexterity and compassion
- Pass a background check and drug test (we are required to test for THC)
- No Covid Vaccination Required
