SUMMARY DESCRIPTION OF WORK

This Program Coordinator will serve as a community liaison and on-site expert for Medication Assisted Treatment Program and SBIRT Training and Implementation. This position may provide supervision and support to staff and will be involved in coordination of services for clients.

Work Schedule: Monday through Friday 8:30 a.m. to 5:00 p.m.

Essential Duties and Responsibilities

  • *Provides regular supervision and support for all program staff as assigned.
  • *Hires supervises monitors trains evaluates and develops program staff.
  • Provides and coordinates direct oversight of admissions to Medication Assisted Treatment.
  • *Networks with community providers and other agency programs in order to assist clients in accessing needed community resources.
  • *Provides outreach and training within the community to include medical professionals community partners and other appropriate populations as needed.
  • *Works as a team member with other program staff to provide trauma-informed care to clients.
  • *Collects Data and completes and submits reports related to SOR/MAT and SBIRT.
  • *Reviews activities and goals with Program Director.
  • *Marketing services within the community.
  • *Supervises the contract budgets along with Program Director and ensures funds are utilized within contract guidelines and sufficiently spent down.
  • *Leads grant contract audits.
  • *Assist clients in ensuring they receive necessary medical care prior to and during the course of medication assisted treatment.
  • *Provide referral and coordination for health issues that may impact the clients ability to participate in medication assisted treatment or health issues related to substance use and mental health.
  • *Provide direct intake/assessment of clients presenting problem and barriers to recovery.
  • *Provide information and referral to address client needs.
  • *Provide problem solving and crisis intervention to clients.
  • *Provide referral and coordination for issues which may interfere with therapy attendance including making follow-up phone calls to clients who miss their appointments.
  • *Maintain all reports required of the position.
  • *The ability to communicate positively with community members regarding coordination of services.
  • *Display a comfort level in working with stressful individual/family dynamics.
  • *Displays a courteous and caring attitude at all times to the clientele volunteers and visitors of the agency.
  • *Cooperates and collaborates with program area staff volunteers and other Agency staff.
  • *Is dependable and punctual regarding scheduling and attendance.
  • Abides by all specific program and Agency procedures policies and requirements.
  • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
    *Creates maintains and shares as appropriate a dynamic self-care plan.
  • *Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agencys ongoing fundraising and friend-raising efforts.
    *Essential functions of this job is to be performed on company physical work site.
  • Performs other program related duties as assigned.

MINIMUM QUALIFICATIONS

  • Bachelors Degree in counseling Social Work or related field.
  • Prior work in or knowledge of Medication Assisted Treatment and SBIRT
  • Valid drivers license acceptable driving record.
  • Training in case management is preferred.
  • Alcohol/drug abuse experience preferred
