Job Summary
Title: Alameda California Role: Occupational Therapist - Home Health Role Responsibilities: Required to prioritize start of care visits Required to supervise Occupational Therapy Assistants Next day mandatory reporting of start of care visits Clinical notes and other documentation due within 48 hours of visit Maintain timely communication with supervisor intake and office Be generally responsive during normal business hoursRequirement: CA OT License and one year of Home Health experience Territory: Alameda Ca (Will be required to expand as needed)Schedule: 40 hours per week Monday-Friday and one weekend a month rotation Training: 3 full days one in house and two in the fieldDocumentation System: KinnserEquipment Issued: iPad Ring Central Mileage Reimbursement: $0.655/mile (from first to last patient)Parking and Toll Reimbursement: PaidCompany Car: Option with a decreased bill rateBill Rate: $90OT Rate: $5 paid after 30 points per week Holiday Rate: $10Weekend Rate: N/AOn-Call Rate: N/ACall Back Rate: N/AProductivity Requirement: 30 points per weekPoint to Hourly Conversion: 1 1.33hrs*Caseloads and visit mixes per day will equal to an average of 8 hrs per day and 40 hrs per week.
