Job Summary
We are seeking compassionate and skilled Registered Nurses (RNs) to join our home health this role you will provide high-quality patient-centered care to individuals in their homes within the Loveland Longmont and Johnstown areas. As a key member of our interdisciplinary team you will assess patient needs develop care plans administer treatments and educate patients and families on managing health conditions. We offer flexible scheduling for full-time or part-time positions allowing you to achieve a healthy work-life balance while enjoying autonomy in your practice. We provide strong support for those new to home health including training on charting and OASIS documentation.

Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans in collaboration with physicians and other healthcare professionals.
- Administer medications wound care IV therapy and other treatments as prescribed.
- Monitor patient progress document care provided and update records in compliance with Medicare and agency standards.
- Educate patients and families on disease management self-care techniques and preventive health measures.
- Coordinate with interdisciplinary team members including therapists and social workers to ensure holistic care.
- Travel to patient homes in the Loveland Longmont and Johnstown areas; maintain accurate mileage and visit logs.
- Participate in on-call rotations as needed (for full-time positions).
- Adhere to all infection control protocols and safety guidelines.

Qualifications
- Active Registered Nurse (RN) license in the state of Colorado (valid and in good standing).
- Associates or Bachelors degree in Nursing from an accredited program.
- Minimum of 1 year of clinical nursing experience; home health experience preferred but not required (training provided).
- Current CPR certification and valid drivers license with reliable transportation and auto insurance.
- Proficiency in electronic health records (EHR) and OASIS documentation.
- Strong communication critical thinking and organizational skills.
- Compassionate empathetic and able to work independently in a fast-paced environment.
- Ability to pass a criminal background check drug screening and TB test.
