Case Manager Social Worker

Schedule: full days Monday - Friday 8am to 4:30 pm. One Weekend Day per month Saturday or Sunday

More about our team

This team assists with discharge planning ensuring patients receive the necessary resources for a smooth transition from the hospital. Youll coordinate with third-party payers government programs and community services to arrange appropriate care housing transportation and other essential services. Your role will involve coordinating placement and support services including long-term care rehabilitation behavioral/mental health home care transportation and DME as well as assisting with CPS APS adoptions and end-of-life planning including advance directives. This position offers an opportunity to make a direct impact on patient care by helping navigate complex discharge processes and fostering continuity of care.

How youll contribute

A Case Manager Social Worker who excels in this role:

  • Interviews and assesses patients and/or patients family caregivers and/or legal representatives.
  • Determines prioritizes provides and/or arranges for needed internal and external services/interventions.
  • Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote monitor and evaluate compliance with patients treatment plan.
  • Assists with discharge planning and processes. Identifies appropriate resources including transportation housing healthcare and social/spiritual services and provides referrals as part of the discharge plan.
  • Mentors new staff students and interns.
  • Performs Medicaid screenings as indicated for NHP.
  • Assists patients with Safety Net applications when needed.

What were looking for

Applicants should have a bachelors degree in social work. Additional requirements include:

  • Master of Science in Social Work preferred
  • NM Social Work license or provisional social work license preferred
