Overview

Shift: Monday-Friday; 8:30 AM - 5:00 PM

The ED BH Diversion Specialist reports to the Supervisor Care Coordination and provides care to clients utilizing the Robert Young Center principles of Trauma Informed Care. This individual will provide comprehensive care coordination services for the population of focus including adults children and/or adolescents with Serious Mental Illness (SMI) Co-occurring disorders (COD) or Severe Emotional Disturbances (SED). This position provides intervention consultation coordinates discharge planning with community-based services and follow clients through the discharge process.

Responsibilities

  • Assures that all clients receive triage/referral and ongoing care coordination services for behavioral health needs.
  • Assist with discharge planning and coordinate with outpatient services.
  • Requests consultation for clients that includes providers social workers and nursing staff when necessary.
  • Delivers high quality care appropriate to the clients individual needs.
  • Completes follow-up connections to offer services to clients who were not met with during shift.

Qualifications

Education:

  • Degree from an accredited nursing program OR
  • Bachelors degree with at least 30 semester hours or equivalent quarter hours in a human services field including but not limited to sociology social work counseling psychology or human services.

License(s)/Certification(s):

  • Current Highest License available as required by degree - must apply within 60 days of hire
  • Valid drivers license required.

Experience:

  • Experience working with persons with severe and persistent mental illness.
