More about our team
Join our team in a 90-bed Skilled Nursing & Rehabilitation Unit with 20 Rehab beds and 70 LTC beds. Staffed with Registered Nurses (RNs) Certified Nursing Assistants (CNAs) and Medication Aides. Operating with a patient ratio of 1:15 days 1:19 evenings and 1:25 nights our unit cares for patients with Congestive Heart Failure (CHF) COPD Diabetes ESRD Renal Failure Dementia Stroke and heart disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs.
How youll contribute
A Registered Nurse (RN) who excels in this role:
- Accurately performs patient assessments and identifies patient needs
- Identifies and initiates appropriate nursing interventions
- Provides care appropriate to condition and age of the patient
- Performs timely and appropriate documentation relating to medical necessity in the medical record
- Responsible for completion and revision of the Interdisciplinary Care Plan for each patient
- Performs timely and accurate QI assessments

What were looking for- a compassionate nurse with a strong foundation in providing holistic and compassionate long-term care with a focus on detailed patient assessment medication administration and chronic disease management.
Applicants should have a current state LPN license and possess a degree from an accredited nursing school. Additional requirements include:
Basic Life Support certification is required within 30 days of hire.
Long-term care (LTC) preferred or desired to work with LTC patients
