DD Services Coordinators provide services to assist individuals and families of individuals with developmental disabilities in selecting obtaining and utilizing resources and services that are available for that addition services coordinators monitor the services provided by foster and sub-contract service provider agencies to assure that services paid for with public funds meet state guidelines for those services.
Duties and Responsibilities
- Assist individuals and families in identifying and engaging in appropriate community-based resources and statewide collaboration with community partners. and in linking with those resources.
- Assess identify plan monitor and evaluate all services for all eligible individuals.
- Collaborate with DHS-Child Welfare for children and with Adult Abuse Investigator for adults to report and perform protective services for individuals and comply with OAR requirements for documenting and reporting.
- Develop a reasonable working knowledge of local and statewide resources and assist in the local development of new resources that serve eligible individuals and families.
- Complete all Oregon Administrative Rule or contract required reports documentation and financial records accurately and in a timely manner.
- Participate in at least 20 hours of relevant training during each year of employment.
- Maintain any and all records of services rendered to clients.
- Attend and participate in all individual clients planning meetings (ISPs and IEPs).
- Ensure participation in facilitation and development of review and monitor each Individual Support Plan (ISP) and services authorized.
- Coordinate and monitor client services including Community First Choice (K Plan) in-home services Family Support Long Term Diversion In-Home Comprehensive and Personal Care 20 services etc.
- Provide limited consultation and education services to clients other agencies and Agency staff.
- Report Serious Incidents as per ODDS definition for data entry as directed by ODDS policy.
- Advocate for clients rights and services.
- Monitor Developmental Disability services including foster care and comprehensive residential and vocational service providers to ensure all Medicaid waivered services are provided according to the service plan and Oregon Administrative Rule (OAR).
- Collaborate with the Designated Referral Coordinator when individuals are in crisis for necessary case management and documentation.
- Coordinate with ODDS staff to provide in or out of home services/placement to individuals.
- Coordinate with ODDS staff to provide crisis diversion services to eligible individuals with developmental disabilities.
- Provide choice counseling upon enrollment and annually thereafter for all adults.
- Complete agency and ODDS data forms.
- Conduct home visits and all necessary out of office meetings which include driving to appropriate destinations.
- Be a Mandatory Abuse Reporter.
- Maintain client confidentiality.
- Other duties as assigned.
Qualifications
Required Education and Experience
- Bachelors degree in behavior science social science or a closely related field; or a bachelors degree in any field and one year of human services related experience
OR
- Associates degree in a behavioral science social science or a closely related field and two years human services related experience
OR
- Three years of human services related experience.
Preferred Education and Experience
- Basic knowledge of developmental disabilities and the developmental disabilities system
- Working knowledge of community resources
Competencies
- Business Acumen
- Communication skills
