Job Description
Ambrose Avenue Group Practice is seeking a motivated and compassionate social prescriber/care coordinator to join our growing team on a full-time basis. This is an exciting opportunity to play a vital role in supporting patients by ensuring they receive the right care at the right time from the appropriate services.
As a social prescriber/care coordinator you will be an integral part of our multi-disciplinary team (MDT) and will work closely with our Primary Care Network (PCN) team. The successful candidate will collaborate with other members of the MDT as needed connecting patients to community groups and statutory services for both practical and emotional support.
Key responsibilities include:
- Collaborate with GPs nurses and other healthcare professionals to support patients with complex health and social care needs.
- Assist patients in navigating the health and care system ensuring they are connected with the most suitable services.
- Provide personalized support to vulnerable individuals and their caregivers.
- Coordinate care for people with long-term conditions frailty and those requiring proactive management.
- Work collaboratively with local organizations to promote positive lifestyle changes.
- Facilitate meetings with organizations to manage and drive new initiatives.
- Offer information and advice regarding a wide range of services to help promote independence within communities.
- Establish strong connections with local organizations community and neighbourhood groups.
- Contribute to reducing the demand on statutory services.
- Work with patients in the practice or in their own homes providing personalized person-cantered support including the use of care technology.
- Help improve patient experience and outcomes within the PCN.
About the Candidate
- Highly organized with strong communication skills.
- Capable of working independently as well as collaboratively within a team.
- Compassionate approachable and focused on patient care.
- Confident in using IT systems and managing information.
- Excellent listening and communication abilities.
- Experience working in healthcare the voluntary sector or community services particularly with vulnerable groups.
- A Diploma Level 2 in Health and Social Care or an equivalent qualification or experience.
- Familiarity with local resources and services and knowledge on how to access them.
- Driven by a desire to help others demonstrating care and empathy.
- Ability to provide person-cantered support in a non-judgmental manner.
- Experience in building relationships with key staff in GP practices attending relevant meetings and engaging with the wider network team while providing information and feedback on the role of social prescribing /care coordinator.
- Understanding the needs of small volunteer-led community groups and the ability to support their development.
- Commitment to collaborative work with all local agencies.
- Experience and confidence in delivering lifestyle change interventions.
