Were seeking a dedicated and reliable Admissions & Social Services Coordinator to join our growing team! This dual-role position is essential in ensuring smooth resident admissions while also supporting the social and emotional well-being of residents and their families. Reliability trustworthiness and consistency with attendance are critical along with strong communication skills compassion and the ability to multitask effectively in a dynamic healthcare environment.
Your Impact as a Admissions & Social Services Coordinator
In this role you will:
Support Admissions: Build trust with residents and families complete bedside assessments and clinically accept residents for placement.
Promote Community Engagement: Represent the facility at community events and make presentations on long-term care and aging to local groups.
Prepare Documentation: Ensure accurate and timely completion of admission paperwork weekly reports and internal documents.
Assist New Residents: Help residents and families adjust during the admission process and provide orientation to the facility.
Develop Care Plans: Create individualized social service care plans to support residents goals and emotional well-being.
Coordinate Support Services: Provide individual and group support to residents and families during times of crisis or transition.
Plan for Discharges: Organize discharge planning to ensure smooth transitions out of the facility.
Facilitate Training: Conduct staff in-services and educational sessions related to social services.
Build Partnerships: Maintain strong relationships with hospitals doctors churches and community agencies to support referrals.
Were looking for someone who:
Has excellent people skills and communicates effectively in writing verbally and in public speaking.
Can work independently multitask and remain composed under pressure.
Holds a Bachelors Degree in Social Work (BSW) or certification as a Social Worker
Designee with relevant education.
Has prior experience in long-term care admissions and/or social services.
Genuinely cares for and is committed to supporting elderly and disabled individuals.
Can travel occasionally and qualifies for company insurance passing DMV requirements.
Complies with the Patient Bill of Rights Employee Responsibilities and all safety policies and procedures.
