The role is situated in the Hoy Center a respected CCRC with a 62 year legacy of serving both residents and short-term rehab patients.

Salary and Benefits

  • Hourly Rate: $45.67 $57.69 (equivalent to $95000 $120000 annually)
  • Bonus: Up to 8% annual bonus potential based on performance

Responsibilities

  • Oversee care quality and nursing standards across assigned long-term care units
  • Serve as the primary RN liaison for interdisciplinary care plan meetings
  • Provide clinical guidance for memory care unit and ensure patient-centered practices
  • Actively participate in Quality Assurance and Performance Improvement (QAPI) committee
  • Ensure all care plans address key clinical risk areas such as falls pressure injuries and chronic conditions
  • Promote dementia care best practices including Montessori and engagement-focused methods
  • Serve as a representative with regulatory agencies families and community stakeholders
  • Partner with Therapeutic Recreation to align resident programming with care needs
  • Lead and support special projects aimed at improving resident outcomes and care delivery
  • Push med carts and provide hands-on nursing support as needed
  • Participate in rotating on-call leadership schedule (approximately one weekend per month)

Requirements

  • Active and unencumbered Registered Nurse (RN) license in Virginia or compact state
  • Minimum 3 years of nursing experience in Skilled Nursing Long-Term Care Assisted Living or Rehab
  • Minimum 3 years of leadership experience (management supervisory or lead RN roles)
  • No direct staff reports but must be capable of leading unit-level clinical operations and initiatives
  • Strong clinical assessment and delegation skills
  • Experience with electronic medical record systems and standard office software
  • Comfortable with collaborative project-based leadership
  • Must be flu-vaccinated; COVID-19 vaccination encouraged per latest CDC guidance
