Job Description
- Facilitate the crisis/exception process with people supported/and their families affiliates and Kansas Department of Aging and Disability (KDADS) staff
- Participate/facilitate complex needs situations with people supported and their families Targeted Case Managers (TCMs) Managed Care Organizations (MCO) and KDADS staff.
- Assist Access Coordinator with transfers in/out gatekeeping and Child in need of Care (CINC) cases.
- Complete functional assessments for special re-evaluations
- Assist with managing access in the new waiver (community support waiver)
Job Requirements
- High school diploma or equivalent
- Bachelors degree in Human Service or related field
- 5 years experience in IDD Field
Experience may be substituted for degree
