Job Summary

**General Responsibilities:**

- Perform health checks and physical exams on new participants as well as regular check-ups.
- Create a medical treatment plan and provide necessary services wherever participants live.
- Talk with participants and their caregivers to understand their needs and help them plan for future care.
- Work together with all team members in a friendly and cooperative environment.
- Help with the nursing care plans for participants.
- Be part of the leadership team to achieve the goals of the organization.
- Work actively with the team sharing knowledge about medical conditions and treatment options.
- Participate in meetings with the team and families to make important decisions.
- Teach participants and their families about health care and managing chronic health issues alongside nursing staff.
- Oversee the healthcare program designed by the team.
- Conduct physical exams order tests and keep track of all results systematically.
- Take part in after-hours on-call duty when required.

**Qualifications:**

- Must have a current physician license to practice in Florida REQUIRED.
- Must be Board Certified in Internal Medicine or Family Practice REQUIRED. Certification in Geriatrics is preferred.
- Experience or additional training in working with older adults is preferred.
- Must have CDS Certification and DEA Registration REQUIRED.
- Previous experience in a supervisory role is REQUIRED.
- At least one year of experience working with elderly or frail individuals is REQUIRED.
- Must have strong skills in assessing physical health and managing chronic diseases in older adults REQUIRED.
- Should have a willingness to work in a creative and different healthcare environment REQUIRED.
- Must be able to adapt to changing priorities based on participant needs REQUIRED.

**Certification Requirements:**

- Board Certified.
- Controlled Substance Registration (CSR/CDS).
- DEA Registration.
- Must have a license in the state of Florida.
