Job Summary
Bury St Edmunds Primary Care Network (PCN) is looking for a motivated and compassionate care coordinator to join our growing team on a full-time basis. This is an exciting opportunity to play a key role in supporting patients across our practices by ensuring they receive the right care at the right time from the right service.
As a care coordinator you will:
- Work closely with GPs nurses and other healthcare professionals to support patients with complex health and social care needs.
- Help patients navigate the health and care system ensuring they are connected with the most appropriate services.
- Act as a point of contact for patients and carers providing personalised support.
- Coordinate care for people with long-term conditions frailty and those requiring proactive management.
- Contribute to improving patient experience and outcomes within the PCN.
About the Candidate
- Highly organised with strong communication skills.
- Able to work independently and as part of a team.
- Compassionate approachable and patient-focused.
- Confident using IT systems and managing information.
- Experience working in the healthcare sector care or community support roles.

BurySt EdmundsPrimary Care Network is a growing PCN with already existing roles including care coordinators social prescriber clinical pharmacists paramedics health and wellbeing coaches and pharmacist technicians that support GPs and other clinical teams to enhance patient services in the local community.
