Job Summary

As our Rapid Access Primary Care for Justice Involved Persons (RA JIP PCP) Physician Associate for Benton and Linn counties Oregon you will provide rapid access to Primary Care for individuals transitioning out of the carceral setting focused on stabilizing the health of the patient during their time of transition. A passion for those experiencing mental health and substance use disorders is required. You will work within a multi-disciplinary team that includes a Program Coordinator dedicated to the program SUD and mental health providers. You will also function as a Rapid Access Provider for established patients of the CHC.

CORE RESPONSIBILITIES

  • Provide rapid access to Primary Care for Justice Involved Persons within the medical home model with a strong focus on prevention disease management and barrier reduction to improve health outcomes. Care will be trauma-informed and patient-centered. It will include medical treatment for substance use disorders including Medication for Opioid Use Disorders. Continuing medical treatment for mental health disorders started in the jail will be necessary (support from psychiatrists readily available). Maintain a primary care relationship until patient has stabilized (usually within 1 year) and can be transferred to a longer-term PCP. (Approximately 50% of the FTE)
  • Provide Rapid Access care for established patients of the CHC (approximately 50% of the FTE).

  • Work in a team that includes certified medical assistants health navigators RN care coordinators clinical pharmacists behavioral health/psychiatric consultants working especially closely with our dedicated Substance Use Disorder Team.
  • Physician supervision will be with the Complex Care Physician. Work side by side in the same hallway with the Complex Care Team and SUD teams. The Complex Care Physician also provides medical care in one of the jails making care coordination even more efficient.
  • Primary care appointments are typically 30 minutes
  • 4 10 hour shifts per week 7 am 6 pm. NO SATURDAYS or SUNDAYS
  • May be required to work at different clinic locations.
  • Limited on-call responsibility (once every 3-4 months) as backup to RN triage service.

  • Annual salary range of:
    • $111616.43 - $163534.66
    • This position is eligible for up to two salary raiseswithin your first year. An Increase may be given at the end of the 6-month probationary period depending on starting salary and at the 1-year anniversary as long as your performance is in good standing! Annual performance-based increases are given until the maximum of the salary range is reached.

Minimum Qualifications for Physician Assistant (HCP5)
The following minimum qualifications are requiredfor this position:
  • Graduated from an accredited Physician Assistant training program
  • Current or ability to obtain a Physician Assistant Unrestricted Oregon Medical License within 3 months of job offer
Applications will be accepted without current Physician Assistant Unrestricted Oregon Medical License but offerand start date will be contingent upon obtaining Medical License. Must be able to obtain Physician Assistant Unrestricted Oregon Medical Licensewithin 3 months of job offer.
Special Requirements:
  • Hiring is contingent upon the successful completion of a background check.
  • This position requires driving and may require use of a personal vehicle.
The Ideal Candidate:
  • Experience providing primary care for patients with multiple barriers to good health (social mental substance use economic housing trauma history).
  • Ability to apply Harm Reduction principles within the boundaries of standards of care/ guidelines.
  • Experience (professional or personal) with Justice Involved Persons.
  • Ability to establish rapport and trust with patients experiencing Systems and other Traumas Severe and Persistent Mental Illness and/ or substance use disorders. Formal training in Motivational Interviewing is a plus.
  • Experience providing Urgent Care including procedural skills consistent with outpatient Urgent Care (basic suturing splinting interpretation of rapid testing rapid assessment of illness). Xray interpretation is NOT required.
  • Ability to maintain appropriate professional boundaries.
  • Strong teamwork skills and passion to work with others to improve systems and reduce disparities.
  • Desire to develop a new program/ team.
  • Ability to thrive with minimal structure/ supervision and is highly flexible and adaptable.
